Shared Housing
Shared Housing Form is used when the parent/guardian and student are living with the home owner or lessee of the property. For shared housing we require the following documentation:
Step 1: You, the parent/guardian, fill out the front page of the Shared Housing Form. You'll need to provide:
- Your driver’s license showing your current address
- A copy of a piece of mail with your name and address on it. This could be a bank statement, phone bill, vehicle registration, paycheck stub, or voter registration card, etc.
- Parent/Guardian signature
Step 2: The homeowner or leaseholder fills out the back page of the Shared Housing Form. They'll need to provide:
- Two (2) of the following documents In the Homeowner/Leaseholder’s name to verify residency:
- Utility company statement showing the parent/guardian name and Isle of Wight address
- Auto/homeowner/rental insurance policy
- Rental contract or rent receipt
- Isle of Wight property tax bill
- Current automobile registration showing the homeowner/leaseholder’s name and Isle of Wight address
- Mortgage verification
- Close of escrow
- Homeowner/Leaseholder’s signature, either notarized or signed in front of the registrar.
If you have lived at this residence for less than 30 days, you will be given temporary enrollment at the time of registration and will have 30 days to provide this required information. Failure to provide required documentation will result in the withdrawal of your child(ren) from school following the 30-day conditional enrollment period.
