Vaccine Interest Survey for Students Age 5-11

Isle of Wight County Schools, in conjunction with Isle of Wight Emergency Services and Rite Aid, is surveying parents to determine the level of interest in their child(ren) receiving the Pfizer vaccine for COVID-19. If you are interested in your child being vaccinated, please complete the brief survey by clicking on the title above. The survey is non-binding. The data from the survey will determine the next steps for scheduling any vaccine clinics for IWCS students. The survey will close Thursday, November 4, at 11:59pm.
