Shared Housing
Shared Housing Form is used when the parent/guardian and student are living with the home owner or lessee of the property. The parent/legal guardian/adult student and the homeowner/leaseholder must complete sections of the form and provide proof of residency. For shared housing we require the following documentation:
Step 1 — Complete the Front-Page Student & Contact Information
• You, the parent/guardian or adult student, fill out the front page of the Shared Housing Form.
Step 2 — Gather Required Documents

(Documents must show your name and current Isle of Wight County address.)

Homeowner/Leaseholder
(Documents must show homeowner/lease holder name and current Isle of Wight County address.)

Step 3 — Complete the Back Page — Statements

- Parent/Guardian/Adult Student completes the top section of the back page of the Shared Housing Form. AND
- Homeowner/Leaseholder completes the bottom section of the back page of the Shared Housing Form.
Parent/Legal Guardian/Adult Student and Homeowner/Leaseholder must:

- Read all statements carefully
- Check all boxes
- Sign and date the form (Both signatures must be witnessed by a notary or Isle of Wight County Schools employees.) Do not sign the form until you are with the notary or IWCS employee.
IMPORTANT NOTES:
- If you moved in within the last 30 days, temporary enrollment may be granted while parent/guardian/adult student documents are collected.
- Missing documents after 30 days may result in withdrawal from school.
- If your housing situation is temporary due to homelessness, ask the school about the McKinney-Vento form instead.
